I strongly suspect this clinic is deliberately sabotaging patients’ testosterone replacement therapy outcomes. It has become routine here to automatically add aromatase inhibitors (anastrozole, etc.) and other “protocol add-ons” to virtually every TRT patient — often at the very first follow-up, long before any evidence of elevated estradiol or actual symptoms (gynecomastia, water retention, mood issues, etc.). Many men are started on these drugs even when their E2 is well within normal reference ranges and they feel great on testosterone alone. This is not evidence-based medicine; it’s blanket pharmacological suppression of a natural and often beneficial conversion pathway. Artificially crushing estradiol routinely blunts or completely negates the mood, libido, joint, brain, and erectile benefits that men seek TRT for in the first place. It feels less like individualized care and more like a deliberate effort to keep patients “managed” rather than truly optimized. Several men I know have left this practice, dropped the unnecessary AI elsewhere, and immediately regained the energy, libido, and sense of well-being they thought TRT was “supposed” to give them. If you want real optimization and not cookie-cutter suppression of basic male physiology, look elsewhere. Very disappointed.
