When you get aroused, your brain sends signals to nerves in your penis to release neurotransmitters like nitric oxide, which helps relax the smooth muscle in the penis’ blood vessels. This allows blood to flow into two chambers and spongy tissue that run along the shaft so they expand.
This expansion compresses the veins to keep the blood in the penis so it stays hard. Anything that interferes with any part of this process can make it difficult to get or keep an erection. To support this, a 2019 review of 10 randomized controlled trials involving 540 patients with ED found that arginine supplementation not only significantly improved erectile dysfunction compared to placebo, but also erectile function, intercourse satisfaction, and orgasmic function (Rhim et al., 2019).
Shockwave therapy
Shockwave therapy is a regenerative treatment for erectile dysfunction that uses pulses of soundwaves to trigger the growth of new blood vessels and improve blood flow. While the technology isn’t news, using it to treat ED is still considered investigational in the U.S. pending more high-quality studies.
Even so, much of the research so far shows it’s safe and effective (Desai et al., 2025), especially for men with mild to moderate vascular ED or those who don’t respond to medications like Viagra (Leslie & Sooriyamoorthy, 2024). It may also provide longer-lasting results than traditional treatments.
A 2026 meta-analysis found that patients who received shockwave therapy reported firmer erections and improved function with no adverse effects (Afdal et al., 2026). In 2021, 40 percent of 425 participants with vascular ED saw treatment success for up to 30 months after shockwave treatment (Adeldaeim et al., 2021).
PRP therapy
Platelet-rich plasma (PRP) therapy is another regenerative treatment for ED that’s still considered investigational but is widely used to treat ED. It involves injecting your own concentrated blood into your penis to help stimulate tissue regeneration and increase blood flow.
A 2025 meta-analysis showed PRP therapy significantly improved erectile function compared to placebo and was even more effective when combined with shockwave therapy. Side effects were rare (Falcone et al., 2026).
In another 2025 study, 35 patients with mild-to-moderate and moderate ED enjoyed improved erectile function for two years after PRP maintenance injections, with no serious side effects (Shaher et al., 2025).
There’s good evidence that PRP therapy may be safe and effective for ED, but more research is needed to understand its long-term effects and to establish clinical guidelines.
Stem cell therapy
Stem cell therapy for erectile dysfunction is a regenerative treatment that involves injecting stem cells taken from your body into the corpus cavernosum, which are the two chambers that run along the length of your penis and fill with blood to create an erection.
Stem cell therapy is believed to promote the repair of the blood vessels, nerves, and other tissues in the penis, but more human studies are needed to confirm its effectiveness as an ED treatment. That said, some research has been promising.
A 2025 meta-analysis found that intercavernosal stem cell therapy improved sexual function, increased satisfaction, led to firmer erections, and improved blood flow confirmed by ultrasound 6 months after treatment. Although results show that stem cell therapy may be a safe and effective treatment for ED, researchers pointed out the need for larger studies (Senel et al., 2025).
What the Evidence Supports
Evidence shows that treating the underlying cause is the best approach to ED treatment. The NIH and other government health organizations recommend a combination of treatments beginning with lifestyle factors aimed at improving metabolic and vascular health.
ED can be improved or prevented through a combination of healthy lifestyle modifications, including exercise, weight management, and nutrition. Diets that prioritize fruits and vegetables, legumes, and sources of long-chain (n-3) fats, such as nuts and fatty fish, can prevent or improve ED and related diseases by improving vascular health and blood flow and lowering inflammation (Bauer et al., 2020).
A 2004 landmark randomized controlled study showed that a diet-and-exercise program improved erectile function in 31 percent of obese men over 2 years, without the use of other ED treatments (Esposito et al., 2004).
Ongoing research on regenerative treatments for ED continues to show promising results, with shockwave therapy and PRP injections becoming more widely available.
Where the Evidence Is Limited
Some treatments, like stem cell therapy, are still in the investigative phase and don’t have enough high-quality research available yet. Further research is needed to determine their long-term safety.
While generally safe when used under the supervision of a doctor, they’re not proven to work for everyone and all treatments come with side effects.
