Pennsylvania
Pennsylvania's ketamine therapy providers cluster around the Philadelphia and Pittsburgh metropolitan areas, with additional access points near Harrisburg and the Lehigh Valley. Patients in rural and smaller communities, including much of northern and central Pennsylvania, generally have fewer local options and should expect to travel toward one of these metros, or occasionally into a neighboring state, for in-person evaluation and treatment.
Ketamine therapy in Pennsylvania follows the same federal and professional framework used nationally. Intravenous ketamine for depression is prescribed and administered off-label, typically by a physician or under direct physician supervision, in a medical office or infusion setting equipped for monitoring. Nasal esketamine is dispensed under a federal risk evaluation and mitigation program that requires observed administration and recovery monitoring on site. Pennsylvania does not impose a distinctive statewide rule on top of these general standards, so scope of practice for nurses, physician assistants, and other staff follows ordinary professional boundaries rather than a treatment-specific state statute.
Before booking, verify a prescriber or supervising physician through the Pennsylvania State Board of Medicine, and verify a nurse through the Pennsylvania State Board of Nursing. Both boards publish license status and public disciplinary history, though neither tracks a clinician's specific ketamine experience or outcomes. Ask who evaluates you, who is present during and after dosing, exactly what is administered and where it is sourced, how risks are disclosed in writing, what happens if treatment does not help, and for an itemized quote covering evaluation, medication, monitoring, and any psychotherapy.

Regulatory context
Ketamine is a DEA Schedule III controlled substance, FDA-approved as a dissociative anesthetic and used off-label for treatment-resistant depression, PTSD, and chronic pain. The FDA approved esketamine (Spravato) in 2019 for treatment-resistant depression under a Risk Evaluation and Mitigation Strategy (REMS) program that requires in-office administration at certified sites. MDMA-assisted therapy remains Schedule I; the FDA issued a Complete Response Letter in August 2024 to Lykos Therapeutics on its MDMA new drug application. Psilocybin is Schedule I and holds FDA Breakthrough Therapy designation through sponsors such as Compass Pathways and Usona, but has not received FDA approval. Oregon Measure 109 (passed 2020, operational 2023) created a state psilocybin service center framework, and Colorado Proposition 122 (2022) authorized regulated healing centers.
Pennsylvania enforces federal scheduling. The Pennsylvania State Board of Medicine oversees prescriber conduct. DEA enforcement on ketamine clinics focuses on diversion and Ryan Haight Act telehealth rules. Pennsylvania PDMP requires controlled substance reporting.
