Autoimmune Disease: Early Signs, Common Types, and When to Get Tested

Your immune system is designed to protect you. It identifies bacteria, viruses, and other foreign invaders, then mounts a targeted response to eliminate them. In autoimmune disease, this system misfires. Instead of attacking only genuine threats, the immune system begins targeting the body’s own healthy tissues.

There are more than 80 recognized autoimmune diseases, and collectively they affect an estimated 24 million Americans, though some researchers place the number closer to 50 million when including conditions still being classified [1]. Women are disproportionately affected, accounting for roughly 78% of all autoimmune disease cases [2].

One of the biggest challenges with autoimmune disease is that it often develops gradually. Early symptoms tend to be vague and overlap with dozens of other conditions, which means the average autoimmune patient sees four or more doctors over several years before receiving a correct diagnosis [3]. Recognizing the early warning signs and knowing which tests to request can shorten that timeline considerably.

What Happens in Autoimmune Disease?

In a healthy immune system, T cells and B cells learn to distinguish between “self” and “non-self” during their development. This process, called immune tolerance, prevents the immune system from attacking the body’s own proteins and tissues.

In autoimmune disease, this tolerance breaks down. The immune system produces autoantibodies (antibodies directed against the body’s own cells) or activates T cells that attack healthy tissue. The specific tissue being targeted determines which autoimmune condition develops. When the attack is directed at the thyroid, it may result in Hashimoto’s thyroiditis. When it targets the joints, it can manifest as rheumatoid arthritis. When it targets the myelin sheath surrounding nerves, it becomes multiple sclerosis [4].

The exact cause of this breakdown varies, but research points to a combination of factors:

  • Genetic predisposition: Certain gene variants, particularly in the HLA (human leukocyte antigen) system, increase susceptibility. Having a family member with an autoimmune condition significantly raises your risk [5].
  • Environmental triggers: Infections, toxin exposures, dietary factors, and stress can activate autoimmunity in genetically susceptible individuals [6].
  • Intestinal permeability: Research by Dr. Alessio Fasano and others has identified increased intestinal permeability (“leaky gut”) as a potential precondition for autoimmune disease development. When the gut barrier is compromised, larger molecules enter the bloodstream and may trigger immune responses [7].
  • Hormonal factors: The strong female predominance in autoimmune disease suggests that sex hormones play a modulating role in immune function [2].
  • Microbiome disruption: Alterations in gut bacteria composition have been linked to multiple autoimmune conditions, suggesting that the microbiome plays a regulatory role in immune tolerance [8].

Early Warning Signs of Autoimmune Disease

Autoimmune diseases can affect virtually any organ or system in the body, so the range of possible symptoms is wide. However, several early warning signs are common across many autoimmune conditions. If you experience several of these simultaneously or persistently, it may be worth investigating further.

1. Persistent Fatigue

Fatigue is the single most common symptom reported by people with autoimmune disease, and it is often the earliest sign. This is not ordinary tiredness that improves with rest. Autoimmune fatigue tends to be deep, persistent, and disproportionate to activity level.

The fatigue of autoimmune disease is driven by chronic inflammation. Pro-inflammatory cytokines such as TNF-alpha, IL-1, and IL-6 directly affect the central nervous system, altering neurotransmitter metabolism and disrupting sleep architecture [9]. Many patients describe it as feeling like they are “running on empty” regardless of how much sleep they get.

2. Joint Pain and Stiffness

Joint pain, particularly when it is symmetrical (affecting the same joints on both sides of the body) and worse in the morning, is a hallmark of several autoimmune conditions. Morning stiffness lasting more than 30 minutes is considered a clinical indicator of inflammatory rather than mechanical joint disease [10].

Joint involvement may be the primary feature (as in rheumatoid arthritis or psoriatic arthritis) or one of several symptoms in a systemic condition like lupus.

3. Skin Changes

The skin is frequently involved in autoimmune disease. Watch for:

  • Rashes that come and go, particularly the butterfly-shaped (malar) rash across the cheeks and nose associated with lupus
  • Dry, scaly patches (as in psoriasis)
  • Unexplained hives or welts
  • Hair loss (alopecia), either patchy or diffuse
  • Sensitivity to sunlight (photosensitivity), where sun exposure triggers rashes or flares
  • Raynaud’s phenomenon: Fingers or toes turning white or blue in response to cold or stress, caused by blood vessel spasms

4. Brain Fog and Cognitive Difficulties

Difficulty concentrating, memory lapses, trouble finding words, and a general sense of mental cloudiness are reported across many autoimmune conditions. Often called “brain fog,” this symptom can be just as disabling as physical symptoms.

Research published in Brain, Behavior, and Immunity has shown that systemic inflammation directly impairs cognitive function by affecting the blood-brain barrier, altering neurotransmitter balance, and activating microglia (the brain’s resident immune cells) [11].

5. Digestive Problems

Gastrointestinal symptoms are common in autoimmune disease, even in conditions not primarily classified as digestive disorders. Symptoms may include:

  • Bloating and gas
  • Abdominal pain or cramping
  • Diarrhea or constipation (or alternating between the two)
  • Food sensitivities that seem to develop suddenly
  • Nausea

Conditions like celiac disease, Crohn’s disease, and ulcerative colitis are explicitly autoimmune-driven GI conditions. But digestive symptoms also appear frequently in lupus, Hashimoto’s, scleroderma, and other systemic autoimmune diseases [7].

6. Recurrent Low-Grade Fevers

A low-grade fever (99.0 to 100.4 degrees Fahrenheit) that comes and goes without an obvious infection may reflect underlying immune system activation. This is particularly common in lupus and adult-onset Still’s disease.

7. Unexplained Weight Changes

Both weight gain and weight loss can signal autoimmune activity. Hashimoto’s thyroiditis often causes weight gain due to slowed metabolism, while Graves’ disease (autoimmune hyperthyroidism) causes weight loss. Celiac disease and inflammatory bowel disease can cause weight loss through malabsorption.

8. Swollen Lymph Nodes

Persistently swollen lymph nodes, particularly in the neck, armpits, or groin, can reflect chronic immune activation. While there are many possible causes, swollen nodes combined with other symptoms on this list warrant evaluation.

Most Common Autoimmune Diseases

Hashimoto’s Thyroiditis

The most common autoimmune condition in the United States, Hashimoto’s involves immune-mediated destruction of the thyroid gland, leading to hypothyroidism. Symptoms include fatigue, weight gain, cold intolerance, constipation, dry skin, and depression. It is diagnosed by elevated thyroid antibodies (TPO and thyroglobulin antibodies) alongside thyroid function tests [12].

Rheumatoid Arthritis (RA)

RA causes chronic inflammation of the joints, particularly the small joints of the hands and feet. Unlike osteoarthritis (which is degenerative), RA is driven by immune activity and can cause joint destruction if untreated. It is characterized by morning stiffness lasting more than 30 minutes, symmetrical joint involvement, and elevated inflammatory markers [10].

Systemic Lupus Erythematosus (SLE)

Lupus is a systemic condition that can affect virtually any organ, including the skin, joints, kidneys, brain, heart, and lungs. It is characterized by periods of flare and remission. The classic butterfly rash, photosensitivity, joint pain, and fatigue are common presenting features [13].

Type 1 Diabetes

An autoimmune attack on the insulin-producing beta cells of the pancreas, Type 1 diabetes typically develops in childhood or adolescence but can appear at any age. It results in absolute insulin deficiency, requiring lifelong insulin therapy.

Multiple Sclerosis (MS)

MS involves immune-mediated damage to the myelin sheath that insulates nerve fibers in the brain and spinal cord. This disrupts nerve signal transmission, causing symptoms such as numbness, weakness, vision problems, balance difficulties, and cognitive changes.

Psoriasis and Psoriatic Arthritis

Psoriasis causes raised, red, scaly patches on the skin due to accelerated skin cell production driven by immune activity. About 30% of people with psoriasis develop psoriatic arthritis, which causes joint inflammation similar to rheumatoid arthritis [14].

Celiac Disease

An immune reaction to gluten (a protein found in wheat, barley, and rye) that damages the lining of the small intestine. Symptoms range from severe GI distress to subtle nutrient deficiencies, fatigue, and neurological symptoms. Some individuals with celiac disease have no digestive symptoms at all.

Inflammatory Bowel Disease (IBD)

Crohn’s disease and ulcerative colitis are the two main forms of IBD. Both involve chronic immune-mediated inflammation of the gastrointestinal tract, causing abdominal pain, diarrhea, bleeding, weight loss, and fatigue.

Testing for Autoimmune Disease

If your symptoms and history suggest autoimmune activity, several laboratory tests can help confirm or rule out the diagnosis.

Antinuclear Antibody (ANA) Test

The ANA test is often the first screening test ordered when autoimmune disease is suspected. It detects antibodies directed against components of the cell nucleus. A positive ANA result does not diagnose a specific condition but indicates that the immune system is producing autoantibodies.

Approximately 95% of people with lupus have a positive ANA, and it is also positive in many other autoimmune conditions. However, 10 to 15% of healthy individuals (particularly women and older adults) may have a low-titer positive ANA without any autoimmune disease [13]. A positive ANA is a starting point, not a diagnosis.

Specific Autoantibody Panels

Depending on the clinical picture, your doctor may order more targeted antibody tests:

  • Anti-dsDNA and anti-Smith: Specific for lupus
  • Anti-CCP (cyclic citrullinated peptide): Highly specific for rheumatoid arthritis
  • TPO and thyroglobulin antibodies: For Hashimoto’s thyroiditis
  • Anti-tissue transglutaminase (tTG-IgA): For celiac disease
  • Anti-SSA/SSB: Associated with Sjogren’s syndrome and lupus
  • Anti-centromere and anti-Scl-70: Associated with scleroderma
  • ANCA (anti-neutrophil cytoplasmic antibodies): Associated with vasculitis

Inflammatory Markers

  • Erythrocyte sedimentation rate (ESR): A nonspecific marker of inflammation
  • C-reactive protein (CRP): Produced by the liver in response to inflammation. More sensitive than ESR for acute inflammation
  • Ferritin: Can be elevated in inflammatory states (in addition to iron overload)

Complete Blood Count and Metabolic Panel

A CBC may reveal anemia (common in chronic inflammation and several autoimmune diseases), low white blood cell count (seen in lupus), or low platelet count. A full metabolic panel helps assess kidney and liver function, which can be affected by autoimmune conditions and their treatments.

Advanced and Functional Medicine Testing

Some practitioners utilize additional tests to evaluate factors that may contribute to autoimmune development:

  • Intestinal permeability testing: Measures markers like zonulin to assess gut barrier integrity [7]
  • Full stool analysis: Evaluates the gut microbiome, digestive function, and inflammatory markers in the gut
  • Food sensitivity panels: While controversial in conventional medicine, some practitioners use IgG food sensitivity testing to identify dietary triggers
  • Environmental toxin panels: Heavy metals, mold toxins, and other environmental exposures can trigger or exacerbate autoimmune activity [6]
  • Organic acids testing: Provides information about metabolic function, nutrient status, and oxidative stress

The Functional Medicine Approach to Autoimmune Disease

Conventional treatment for autoimmune disease typically focuses on suppressing the immune response with medications such as corticosteroids, disease-modifying antirheumatic drugs (DMARDs), and biologics. These treatments can be effective at controlling symptoms and preventing organ damage, and they remain important tools, especially in moderate to severe disease.

Functional medicine takes a complementary approach by asking why the immune system became dysregulated in the first place and addressing those root causes [15]. This typically involves:

Identifying and Removing Triggers

  • Dietary triggers: Gluten, dairy, and other foods may drive inflammation in susceptible individuals. Elimination diets or targeted testing can help identify problematic foods.
  • Environmental toxins: Heavy metals, mold, pesticides, and other environmental exposures can activate or worsen autoimmune responses.
  • Chronic infections: Certain infections (Epstein-Barr virus, Lyme disease, H. pylori) have been associated with autoimmune disease onset [6].
  • Stress: Chronic psychological stress activates the HPA axis and shifts immune function toward autoimmune-promoting pathways.

Healing the Gut

Given the connection between intestinal permeability and autoimmune disease, many functional medicine protocols prioritize gut repair. This may include:

  • Removing inflammatory foods and potential allergens
  • Restoring digestive function with enzymes and stomach acid support
  • Reinoculating the gut with beneficial bacteria through probiotics and prebiotic-rich foods
  • Repairing the gut lining with nutrients like L-glutamine, zinc, and omega-3 fatty acids [7]

Reducing Inflammation

  • Anti-inflammatory diet: Emphasizing vegetables, fruits, wild-caught fish, olive oil, nuts, seeds, and herbs while minimizing processed foods, refined sugar, and seed oils
  • Omega-3 fatty acids: EPA and DHA have documented anti-inflammatory effects and have been studied in rheumatoid arthritis, lupus, and IBD
  • Curcumin: The active compound in turmeric has been shown to modulate multiple inflammatory pathways
  • Vitamin D: Deficiency is strongly associated with increased autoimmune risk. Supplementation to optimal levels (40 to 60 ng/mL) is commonly recommended [15]

Supporting Immune Regulation

  • Adequate sleep: Sleep deprivation disrupts immune regulation and increases inflammatory markers
  • Stress management: Meditation, yoga, and other stress-reduction practices can help modulate immune function
  • Regular exercise: Moderate exercise has anti-inflammatory effects, though overtraining can worsen autoimmune symptoms
  • Toxin avoidance: Reducing exposure to environmental chemicals that may disrupt immune function

When to See a Doctor

You should seek medical evaluation if you experience:

  • Persistent fatigue that does not improve with rest or lifestyle changes
  • Joint pain and morning stiffness lasting more than 30 minutes
  • Unexplained rashes, hair loss, or skin changes
  • Multiple symptoms from the early warning signs list occurring simultaneously
  • A family history of autoimmune disease combined with suggestive symptoms
  • Numbness, tingling, or weakness in your extremities
  • Unexplained fevers, weight changes, or swollen lymph nodes
  • Digestive problems that do not respond to standard treatments

Early detection matters. Many autoimmune conditions cause progressive tissue damage, and starting treatment before significant damage occurs leads to much better long-term outcomes. If your primary care physician is not responsive to your concerns, consider seeking a rheumatologist, endocrinologist, or functional medicine practitioner with experience in autoimmune conditions.

The Bottom Line

Autoimmune disease is common, underdiagnosed, and often misunderstood. The early symptoms, including fatigue, joint pain, brain fog, skin changes, and digestive issues, can be easy to dismiss individually but become significant when they appear together or persist over time.

Getting tested is straightforward. An ANA test, inflammatory markers, and specific autoantibody panels can provide clarity relatively quickly. And while autoimmune disease cannot be “cured” in the conventional sense, it can often be managed effectively and sometimes brought into remission through a combination of conventional treatments and a functional medicine approach that addresses root causes like gut health, dietary triggers, environmental exposures, and stress.
